- Information about Your Spouse: His/Her Last Name/Surname, First Name and Middle Name, Gender, Date of Birth, Country of Birth, Native Country.
- Information about Your Child/Children under 21 Years of Age: Number of children, Name, Date of Birth, Country of Birth.
